Diabetes Education

Research shows that lifestyle modifications, including diet and exercise, are the most effective things you can do to regulate your blood sugar. Southwest Memorial Hospital offers a nationally recognized and approved Diabetes Self Management Education program designed to provide individuals who have diabetes with knowledge and skills to help them take control of their health.

Services include:

One-on-one sessions with a Certified Diabetes Educator (CDE) tailored to meet the needs of the individual.
Group sessions covering a variety of diabetes-related topics.
Food preparation classes and cooking demonstrations.
Monthly diabetes support group meetings.

The Diabetes Education Department at SWMH is open Monday through Friday from 8:00 a.m. until 5:00 p.m. Some services are covered by insurance and some are offered free of charge.

Nutrition Education

SHS’s Nutrition Education department provides diabetes education and nutrition services based on current clinical research. Using an integrative approach, both one-on-one and group sessions are tailored to the individual’s needs.

One of the most important decisions we all make each day relates to our food choices, as asll as other lifestyle factors such as physical activity and stress management. The registered dietitian professionals at SHS are committed to empowering patients to take responsibility of their health by making healthy lifestyle choices.

Services include:

Inpatient nutrition and diabetes assessment and education
Diabetes self-management
Outpatient medical nutrition therapy and education
Cardiopulmonary rehabilitation nutrition education
Weight magagement, including pre-bariatric surgery counseling
Home health visits
Grocery shopping tours for patient groups
Community outreach to schools/organizations
Eat Healthy/Eat Local program targeting local food establishments
Diabetes support group
Food preparation classes and cooking demonstrations

Individuals may receive services after a referral has been made by their healthcare provider. If you are a patient, physician, or community organization interested in hosting an SHS sponsored nutrition event, you may reach us at 970-564-2276 from 8:00 a.m. until 4:30 p.m. Services are either covered by insurance or are provided complimentary as part of our community outreach effort.
