Southwest Memorial Hospital offers a nationally recognized and approved Diabetes Self Management Education program designed to provide individuals who have diabetes with knowledge and skills to help them take control of their health.
One-on-one sessions with a Certified Diabetes Educator (CDE) tailored to meet the needs of the individual.
Group sessions covering a variety of diabetes-related topics.
Food preparation classes and cooking demonstrations.
Monthly diabetes support group meetings.

The Diabetes Education Department at SWMH is open Monday through Friday from 8:00 a.m. until 5:00 p.m. Some services are covered by insurance and some are offered free of charge.
Nutrition EducationSHS’s Nutrition Education department provides diabetes education and nutrition services based on current clinical research. Using an integrative approach, both one-on-one and group sessions are tailored to the individual’s needs.

Individuals may receive services after a referral has been made by their healthcare provider. If you are a patient, physician, or community organization interested in hosting an SHS sponsored nutrition event, you may reach us at 970-564-2276 from 8:00 a.m. until 4:30 p.m. Services are either covered by insurance or are provided complimentary as part of our community outreach effort.
