Facing an injury
or illness can be a difficult time for patients and families. Often,
there are unforeseen circumstances that arise that make the situation
more stressful. Confronting tasks such as lining up care at home and
managing medical and prescription costs can be overwhelming when struggling
with illness or recovery. Southwest Memorial Hospital offers social
work services to help meet these and other social and emotional needs
of patients and their loved ones in both the hospital and outpatient
settings.
Through interactions
with the entire health care team, the social services worker identifies
patients with potential needs. The social services worker provides information
on available community resources to help patients and families maintain
the safest, most independent lifestyle possible. Interventions may vary
from lining up transportation, assisting with financial concerns, or
arranging for care at home, a rehabilitation unit or in a nursing home.
