Diabetes Program

Research shows that lifestyle modifications including diet and exercise is one of the most effective things you can do to lower your blood sugar. Southwest Memorial Hospital offers a nationally recognized and approved Diabetes Self Management Education program designed to provide individuals who have diabetes with knowledge and skills to help them take control of their health.

We offer group and individualized sessions that highlight nutrition, physical activity, monitoring, medication, reducing risks and healthy coping and problem solving. Ask your doctor if Diabetes Self Management Education classes are right for you.
