is proud to offer Home Health Services to the people of Montezuma County. Each
staff member is deeply committed to patient care, attentive to patient's
personal preferences, individual care needs and rights of choice.
During those times when you
find that you and your family are unable to meet all your healthcare needs, let
assist by providing a little extra help. The nurse and staff of Southwest
Memorial Hospital Home Health Services work closely with both patients and
physicians to plan the care to on an individual basis. The involvement of the
patient in his/her own care leads to a greater feeling of control and well-being
over their life, along with a sense of dignity of being their own home.
Who is Eligible for Home Health Care?
In most instances, the cost of Home Health Services are covered under private
insurance and, when specific criteria is met, by Medicare and Medicaid. To be
eligible for Home Health, the following criteria must be met:
You must be homebound due to sickness or injury which makes it difficult to leave home in order to receive medical care/treatment.
You must be under a physicians care and the physician must order the services.
You must require
a skilled, intermittent service such as skilled nursing care, physical therapy
or speech therapy

Note: Medicare patients must
be receiving a skilled service in order to be eligible for personal care.

strengthen muscles

walking and transferring from chair to bed

Medical Equipment and Supplies
Includes assistance with the arrangements, supplies and equipment needed to meet
the patient's individual needs.
