What is the Patient Portal?
The Patient Portal is a free, secure online tool that allows you to review contents of your Southwest Medical Group medical record and communicate with your provider(s). These services are available 24/7 online for your convenience.
What do I need to use the Portal?
You must be an established Southwest Medical Group patient and will need an email address, an internet connection, and a unique one-time password from your Southwest Medical Group office, which will be used only to setup your account. You will be prompted to setup your your own password and security questions at this time.
What if I don’t have an email address?
An email address is required to create an account on the Southwest Medical Group Patient Portal. If you do not have one, you may create an email account free of charge from various sources online. We have listed some email providers below:
How Do I Sign Up?
Please visit your Southwest Medical Group office and a staff member will enroll you in the portal. At that time, you will receive a letter with this unique one-time password. Once you have this letter, please visit myhealth.swhealth.org and select login using your email address and one-time password. Upon successful sign-in, you will be prompted to create your own secure password and setup security questions. Once you have completed this process, you can bookmark the myhealth.swhealth.org website and begin using the Patient Portal to review your health records.
Please note, the information you enter when verifying your identity must match EXACTLY what we have in your record.
What information is available?
You can view information* from your Southwest Medical Group Electronic Medical Record including some test results, medication and problem lists, allergies, upcoming and past appointments. You will also be able to send and receive secure messages from your physician’s office, as well as print, download, or send your Chart Summary.
*Please note, information from your Southwest Memorial Hospital visits will not be included in this portal. This is due to the hospital having a different health record system than we have in the clinics.
Is there an age requirement to use the portal?
Yes, individuals must be at least 18 years old* to participate in the Southwest Medical Group Patient Portal.
*Exception applies only to emancipated minors with supporting documentation.
Can I see my family members’ medical record information through the portal?
At this time, we are not allowing patients to review their family members’ records via the Patient Portal.
What if I need more information than is available through the portal?
If you need a more complete record, please contact our Health Information Management Department at (970) 564-2311 or visit your Southwest Medical Group office and request a hard copy of your Medical Records.
What if I can’t remember my password?
On the portal homepage, there is an ‘I forgot my password’ link in the Log-in box. Select that link, and the system will prompt you for your email address. Type your address and select ‘Reset Password’; the system will then ask you to answer the two security questions that were chosen by you when you setup your account. Type your answers and submit. As long as your answers are correct, you will be asked to create a new password. If you need further assistance, please call your Southwest Medical Group office.
