Board Member Candidate Application

Thank you for your interest in serving on the Southwest Health System Board of Directors! Serving through Board Governance can be challenging and rewarding. The SHS board anticipated time commitment averages 8-10 hours per month for board development, education, committee meetings, and board meetings.

Please complete the application form, the Conflict of Interest form, as well as provide your resume/CV for review by our Nominating Committee. The application and Conflict of Interest Form can be found below.

You will be contacted following the closing of the application cycle for the week following September 12th. If chosen for an interview, your interview will be scheduled for the week(s) of 9/22 or 9/29 only. If you are not available during these weeks for a short interview in the evening, we may not be able to consider your application this year.

We encourage you to stay engaged with the Southwest Health System and to attend board meetings when you are able to do so. In the meantime, should you have general questions, many will likely be answered by reviewing our board information at https://www.swhealth.org/about-us/boards/southwest-health-system/.
