April 5, 2021: Cortez, CO – Southwest Health System, Inc. (SHS) announces Carly Temple, FNP, joins Southwest Medical Group complementing our current team of providers serving on our main campus SMG Primary Care and Pulmonology.

“Carly will join two teams at SMG- Pulmonary and Family Practice. We are very excited to add her expertise to our team and continue to provide quality care close to home.” Kerri White-Singleton, COO

She is Board-Certified through American Association of Nurse Practitioners and completed her FNP Master’s Program at Wilkes University in Family Medicine and Hematology/Oncology. She began her career in 2013 and has developed her skills through a broad spectrum of care modalities including Neonatal ICU, OB, Hematology/Oncology and Infusion in large and rural health care systems.

She says of her practice, “My favorite part about medicine is interacting with patients, learning their stories and supporting them with evidenced-based best practices and being a safe resource for patients and their families. I work to constantly evolve my practice and include holistic healthcare and lifelong learning and find this to be extremely fulfilling as a primary care provider.”

Carly was born in Colorado and although much of her nursing career has been in South Dakota, she is thrilled to move her son Oliver back to the area to be close to her parents and her sister. She loves exploring the outdoors in our area and says of SW Colorado, “I remember wonderful trips to Mesa Verde, Lake Powell and Moab as a child. The red dirt, soaring mountains and peaceful country make this a desirable location to live. I want to live somewhere that I don’t have to take a vacation from. I want my son to grow up surrounded by a tight-knit community.”
